Community Health Assessment Available Online

San Juan Basin Health Department announces the availability of the 2012 Community Health Assessment for Archuleta and La Plata Counties online at sjbhd.org/communityhealth.

This Community Health Assessment was conducted to identify health issues of primary concern and to provide critical information to those in a position to make an impact on the health of our region.  It is the first step in developing a Public Health Improvement Plan. San Juan Basin Health partnered with Mercy Regional Medical Center and Pagosa Springs Medical Center in producing the assessment.
